Relay Withdrawal/Leave of Absence Form

The Registrar's Office is here to help you through the withdrawal or leave of absence process at Relay. Please know that your withdrawal or leave of absence request is not marked as final until you receive a confirmation email that it has been processed. It normally takes up to five business days for withdrawals to be processed and up to ten business days for leave of absences to be processed.

Are you a Relay Resident? (A student who is currently or was previously enrolled in deliberate practice). *This question is required.
We highly recommend you consult with your advisor and then sign up for an appointment with the Student Financial Service team before completing this form. Leaving Relay can result in significant financial and employment implications. 
  • If you have anticipated AmeriCorps Segal Education Awards or anticipated Residency Scholarship, you will have to immediately pay back your deferred tuition once you receive an email from Relay confirming that your withdrawal  or leave of absence has been processed.
  • Please refer to your billing statement to know if this applies to you.
